The science

How accurate is PCR STI testing compared to rapid tests?

PCR (polymerase chain reaction) detects the actual DNA of pathogens, making it significantly more accurate than rapid antibody tests. PCR can identify infections within days of exposure. often weeks before antibody tests would show positive.

Most pharmacy "rapid tests" look for antibodies, your body's immune response to an infection. The problem? Antibodies can take 2-12 weeks to develop after exposure. During that window, a rapid test may show negative even when you're infected and contagious.

Epicentre uses laboratory-grade PCR technology. the same molecular testing used by hospitals and research institutions. PCR amplifies and detects the pathogen's DNA directly, catching infections at their earliest stage.

"PCR testing has transformed sexual health diagnostics. We routinely detect chlamydia and gonorrhoea infections that would be missed by rapid tests for weeks. Early detection means earlier treatment. and less transmission to partners."
Dr. Samantha Naidoo, MB ChB, FCP (SA), Medical Director, Epicentre
What PCR detects that rapid tests miss: Asymptomatic chlamydia, early-stage gonorrhoea, Mycoplasma genitalium (emerging STI), Ureaplasma, Trichomonas vaginalis, and precise HSV-1/HSV-2 strain identification. PCR also distinguishes between active infection vs. past exposure. → See testing window periods for each infection

The reality

Why is STI testing so important in South Africa?

South Africa has one of the highest STI burdens globally. Understanding the scale helps explain why regular testing isn't optional. it's essential self-care.

STI Prevalence in South Africa

NICD Data 2021-2024
Chlamydia
Chlamydia trachomatis
14.7% women / 6% men
Most common bacterial STI. Usually symptomless. Untreated, it causes pelvic inflammatory disease and infertility. Curable with antibiotics from your doctor.
Gonorrhoea
Neisseria gonorrhoeae
6.6% women / 3.5% men
Rising antibiotic resistance. SA guidelines now require dual-antibiotic treatment. Can cause infertility if untreated. Often co-occurs with chlamydia.
Herpes (HSV-2)
Herpes simplex virus type 2
43.6% of genital ulcers
Lifelong but manageable. Causes genital sores. Increases HIV transmission risk. Antiviral medication controls outbreaks.
Trichomonas
Trichomonas vaginalis
7.6% women / 1.7% men
Often missed. Causes vaginal discharge and irritation. Easily cured with treatment from your doctor. Men frequently asymptomatic carriers.
Syphilis
Treponema pallidum
30.2% of genital ulcers
Resurgent in SA. Progresses through stages if untreated. Fully curable with treatment. Critical to test in pregnancy.
HIV
Human immunodeficiency virus
~13% national prevalence
Manageable with treatment. Modern antiretrovirals achieve undetectable = untransmittable (U=U). Early detection is key.

Real Questions We Get Every Day

Sister Lucia (Parktown North) and Sister Lindiwe (Observatory) answer the questions patients actually ask. the ones you might be too embarrassed to Google.

"Can I test anonymously? I don't want this on my medical aid."
Sister Lucia: Absolutely. Just say "I'd like to test anonymously" when you check in. Give us any name you like and a private email. Pay cash or card. no medical aid claim, no SMS to anyone, no paper trail. Your results go only to you.
"I'm too embarrassed to say 'STI test' out loud in the waiting room."
Sister Lindiwe: You never have to say those words. Just ask for "the Express Package" or show us the name on your phone. We do all intake in a private room. nobody in the waiting area hears anything.
"Can my partner come with me for support?"
Sister Lucia: Yes! Many couples test together before ditching condoms. You can be in the room together or separately. your choice. Just let us know when you arrive.
"I had unprotected sex last night. Can I test today?"
Sister Lindiwe: Testing immediately won't give accurate results. infections need time to become detectable. For PCR tests, wait at least 3-5 days. For blood-based antibody tests, 2-4 weeks is ideal. Come see us, and we'll advise on the right timing for your situation.
"What if I test positive? Will you judge me?"
Sister Lucia: Never. We see this every single day. you're not the first and won't be the last. Most bacterial STIs are curable with one dose of antibiotics. We'll connect you with a doctor who can prescribe treatment, usually the same day. No lectures, just solutions.
"I need results urgently, like tomorrow. Is that possible?"
Sister Lindiwe: For our Express Package, yes. Tell us you need "urgent results" when you check in, arrive before 11am, and we'll prioritise your sample. Results often come back within 24 hours. PCR panels take longer (48-72 hours urgent), but we'll do everything we can.
