The science

How accurate is PCR STI testing compared to rapid tests?

PCR (polymerase chain reaction) detects the actual DNA of pathogens, making it significantly more accurate than rapid antibody tests. PCR can identify infections within days of exposure, often weeks before antibody tests would show positive.

Most pharmacy "rapid tests" look for antibodies, your body's immune response to an infection. The problem? Antibodies can take 2-12 weeks to develop after exposure. During that window, a rapid test may show negative even when you're infected and contagious.

Know the signs

STI symptoms in women and men

Many STIs show no symptoms, so testing is the only way to be sure. When signs do appear, what you notice depends on your anatomy more than your gender, so the signs below apply whatever your gender identity.

Unusual or discoloured discharge
Yellow, green or grey discharge, or a new smell. Common with chlamydia, gonorrhoea, trichomoniasis or BV.
Sores, ulcers or a painless sore
A painless sore can be early syphilis (read the signs); painful blisters point to herpes.
Burning or pain when you pee
Often chlamydia or gonorrhoea, and a common sign of a UTI.
Itching, rash or genital warts
Herpes, HPV, or a body rash that can come with syphilis.
Pelvic or lower-tummy pain
Untreated chlamydia or gonorrhoea can move deeper and cause pelvic inflammatory disease.
Often nothing at all
HIV, chlamydia and HPV can stay silent for years. Testing is the only way to know.

"Can I test anonymously? I don't want this on my medical aid."
Sister Lucia: Absolutely. Just say "I'd like to test anonymously" when you check in. Give us any name you like and a private email. Pay cash or card. No medical aid claim, no SMS to anyone, no paper trail. Your results go only to you.
"I'm too embarrassed to say 'STI test' out loud in the waiting room."
Sister Lindiwe: You never have to say those words. Just ask for "the Express Package" or show us the name on your phone. We do all intake in a private room. Nobody in the waiting area hears anything.
"Can my partner come with me for support?"
Sister Lucia: Yes! Many couples test together before ditching condoms. You can be in the room together or separately, your choice. Just let us know when you arrive.
"I had unprotected sex last night. Can I test today?"
Sister Lindiwe: Testing immediately won't give accurate results. Infections need time to become detectable. For PCR tests, wait at least 3-5 days. For blood-based antibody tests, 2-4 weeks is ideal. Come see us, and we'll advise on the right timing for your situation.
"What if I test positive? Will you judge me?"
Sister Lucia: Never. We see this every single day. You're not the first and won't be the last. Most bacterial STIs are curable with a single course of prescription treatment. We'll connect you with a doctor who can prescribe treatment, usually the same day. No lectures, just solutions.

Do I need to fast before an STI test?

No fasting required for STI tests. Eat and drink normally before your appointment. (Fasting is only needed for glucose/cholesterol tests.)

What is an STI, and is it the same as an STD?

An STI (sexually transmitted infection) is an infection passed on through sexual contact. STD is an older word for the same thing once it causes symptoms. Common STIs in South Africa include chlamydia, gonorrhoea, syphilis, herpes, HPV and HIV. Many cause no symptoms, so testing is the only way to know. See our full STI guide.

Are STIs curable?

Many are. Chlamydia, gonorrhoea, syphilis and trichomoniasis are bacterial and fully curable with a course of prescription treatment from a doctor. Herpes, HPV and HIV are not cured but are well managed with treatment. If your result is positive, Test and Treat connects you to a doctor, often the same day.

Is there free STI testing in South Africa?

Public clinics offer free HIV and STI testing through the Department of Health, though queues and a limited test menu are common.
