What Does a Full Thyroid Panel Include?

TSH (Thyroid Stimulating Hormone): the brain's signal to the thyroid. High TSH suggests underactive thyroid. Low TSH suggests overactive. This is the screening marker most doctors use, but it is not enough alone.

Free T4 (Thyroxine): the main hormone the thyroid produces. It must be converted to T3 to be used by cells. Shows thyroid output.

Free T3 (Triiodothyronine): the active form of thyroid hormone. What your cells actually use for energy. Often low even with normal TSH. Factors affecting conversion include stress, iron deficiency, inflammation and gut health.

Thyroid antibodies (TPO-Ab and TG-Ab): detect autoimmune thyroid disease (Hashimoto's, Graves') before TSH changes. Critical for a complete picture.

What is a full thyroid panel?
TSH (brain's signal), Free T4 (thyroid output), Free T3 (active hormone) and thyroid antibodies (TPO and TG for autoimmune detection). Many doctors only test TSH, but this misses conditions where TSH is normal but T3 is low or antibodies indicate early autoimmune disease.
Do I need to fast?
No fasting for the thyroid panel. If your panel also includes glucose or insulin (like ODx Max), fast 8 – 10 hours. On thyroid medication? Test in the morning before your dose.
Why test antibodies if my TSH is normal?
Thyroid antibodies can be elevated for years before TSH changes. This is especially true with Hashimoto's. Catching autoimmune thyroid disease early allows for lifestyle interventions that may slow progression.
What is the difference between T4 and T3?
T4 is the main hormone the thyroid produces, but it is inactive. The body converts it to T3, the active form cells use. Some people have normal T4 but poor conversion to T3. Testing both reveals this. Conversion is affected by stress, iron deficiency, inflammation and gut health.
Can thyroid problems cause weight gain?
