Vitamin C and colds: what the evidence actually says
Seventy years of argument, thousands of people in trials, and a conclusion that pleases nobody. Vitamin C will not stop you catching a cold. It will shorten one slightly, but only if you were already taking it. And none of that research is about flu.

Does vitamin C stop you catching a cold?
No, not if you are an ordinary healthy adult. A Cochrane review pooled 29 trial comparisons covering 11,306 people and found that taking vitamin C regularly made no meaningful difference to how often they caught colds. This is the single most tested claim in the vitamin world, and it does not hold up.
The idea comes largely from Linus Pauling, a Nobel laureate who argued in 1970 that large doses would substantially reduce colds. It was a serious hypothesis from a serious scientist, and it prompted decades of careful trials. Those trials did not find what he expected.
It is worth being precise about what "no effect" means here. The trials were not small, they were not badly run, and the result is not borderline. Here is the whole evidence base in one view.

Read the badges down the left. One yes, and it belongs to marathon runners and soldiers rather than to anyone buying tablets for the office. The first row is the one that settles the popular claim: eleven thousand people is a lot of people to find nothing in.
What vitamin C actually does
Three things it does well, none of which are what it is sold for. It is the cofactor that lets your body build collagen, which is why deficiency shows up as poor wound healing and bleeding gums. It converts dietary iron into the form your gut can absorb. And immune cells concentrate it far above blood levels, which is why deficiency does impair how they work.
That last point is worth separating carefully. Deficiency impairing immune function is not the same claim as supplementation improving it in someone who already has enough. The first is well established. The second is what the cold trials tested, and mostly did not find.
The iron connection is the useful one. Vitamin C makes non-haem iron, the kind in plants, considerably easier to absorb. If you are tired and taking vitamin C hoping it helps, the mechanism most likely to be doing something is that one, and it is worth knowing whether your iron is actually low before guessing.
Does vitamin C shorten a cold?
Slightly, and only if you were already taking it before the cold began. Pooled across 31 comparisons and 9,745 cold episodes, regular supplementation shortened colds by about 8% in adults and 14% in children. A separate 2023 meta-analysis found symptoms were about 15% less severe.
One detail matters more than the headline figure: the benefit shows up most clearly in people whose vitamin C intake was low to begin with. That points at correcting a shortfall rather than topping up someone who already has enough, which is a different proposition from the one on the packaging.
Eight per cent sounds better than it feels. Here is what it looks like on a seven-day cold.
Thirteen hours, for taking something every day of the year. In children it is closer to a full day, which is a more interesting number. Whether either is worth it is a judgement call rather than a medical one, and it is reasonable to land on either side of it.
The part most people get wrong. Reaching for vitamin C when you feel a cold coming on is the most common way it gets used and the version with the least support. Trials that started high doses after symptoms had already appeared found no consistent effect on duration or severity. The modest benefit in the research comes from having taken it beforehand.
A cold is not flu, and the research does not cover flu
All of the evidence above concerns the common cold. Influenza is a different illness caused by a different family of viruses, and none of these findings should be read across to it.
The practical difference is mostly in how it arrives.

Colds are miserable. Flu can be dangerous, particularly for older people, during pregnancy, and for anyone with a long-term condition.
If you are unwell enough to be asking. Vitamin questions are for when you are well. If you have a high fever, breathlessness, chest pain, confusion, or symptoms that improve and then sharply worsen, that is a doctor's visit rather than a supplement decision. The one group where it does prevent colds
There is a real exception, and it is oddly specific: people under short bursts of extreme physical stress. Across five trials in 598 marathon runners, skiers and soldiers on subarctic exercises, vitamin C roughly halved the risk of catching a cold.
That is a large effect and it was consistent across all five trials, which is why it survives in the literature while the general population finding does not. Note the doses were not exotic, between 0.25 and 1 gram a day, so the difference is not explained by taking more. Something about extreme exertion, or the cold environments that usually came with it, changes the picture.
A more recent and larger trial points the same way. In 2019, 1,444 South Korean army recruits in basic training were randomised to vitamin C or placebo for 30 days, and the vitamin C group developed fewer colds. Basic military training is the same kind of sustained physical stress as the earlier trials, and the finding held in a group many times larger.
Whether an ordinary week of gym sessions counts is not established. The people in these trials were running marathons, skiing competitively or working outdoors in subarctic conditions. If you are training at that level, the evidence is on your side. If you are not, it probably does not apply to you.
Can you test your vitamin C levels?
It is possible, but rarely useful, and Epicentre does not offer it as a routine test. A blood vitamin C level mostly reflects what you ate in the last day or two rather than your longer-term status, the sample degrades quickly once drawn, and outright deficiency is uncommon where fresh produce is available.
Saying that costs us a sale, which is the point. A test is only worth paying for when the result changes what happens next. A vitamin C level would tell you roughly what your last two days of eating already tell you, and it would not change any advice you would be given.
There is a version of the question worth taking seriously. Genuine vitamin C deficiency does exist and it does affect how the body heals and defends itself. But it shows up in specific circumstances: very restricted diets, heavy alcohol use, some digestive conditions, or long periods without fresh fruit and vegetables. If that describes you, the conversation is with a doctor about your diet, not with a laboratory about a number.
The vitamins worth testing instead
Vitamin D, vitamin B12 and iron. Each is commonly low, each produces vague symptoms that are easy to blame on something else, and each is simple to correct once you know. Unlike vitamin C, a blood level for these genuinely tells you something you could not otherwise know.
Tiredness, low mood, poor concentration and getting run down are the symptoms that send people to the vitamin C aisle in the first place. They are also the classic presentation of low iron, low B12 and low vitamin D. Testing tells you which, if any, you are actually dealing with, and it is cheaper than a year of guessing.

Where this lands in South Africa
Two things make the vitamin C question play out differently here. Supplements are easy to buy and blood tests are not, and the cost of a year of guessing is often higher than the cost of finding out.
A pharmacy aisle of high-dose vitamin C and combination immune products is available to anyone with cash, in every town, with no consultation required. A blood test has traditionally meant a GP visit first, a referral, then a laboratory account, which is both slower and more expensive than the supplement. That asymmetry is why so many people end up treating a guess rather than a finding.
There is a second South African story running underneath the first. Vitamin C deficiency is not a historical curiosity: it is re-emerging internationally, and the groups at risk are consistent everywhere, being people with limited access to fresh produce, older people, heavy drinkers, smokers and people with chronic illness. Where fresh fruit and vegetables are expensive or hard to reach, genuine deficiency is plausible. So the country has both problems at once: people spending money on vitamin C they will excrete, and people who cannot easily buy an orange.
Walk-in testing changes the arithmetic. A vitamin D test at R300 and a B12 test at R206 cost less than several months of a mid-range supplement, and you need no doctor's referral if you are paying cash. If you intend to claim from a medical scheme you will generally need a referral with an ICD-10 code, which is worth knowing before you assume the claim will go through.
