Gamma GT is an enzyme found mainly in the liver and bile ducts. It's one of the most sensitive markers of bile duct strain and it responds to alcohol, which is why it's usually read alongside ALT and alkaline phosphatase rather than on its own. What does Gamma GT measure?
Gamma-glutamyl transferase sits on the surface of cells lining the liver and the bile ducts. When those cells are irritated or the flow of bile is obstructed, the enzyme leaks into the blood.
It's sensitive, which is its strength and its weakness. Gamma GT picks up problems early, and it also rises for reasons that turn out to be nothing. On its own it's rarely a diagnosis. What it does well is tell a doctor which of the other liver markers to take seriously.
What does a high or low Gamma GT mean?
Before reading anything into either, remember how reference ranges work. They're built so that 95% of healthy people fall inside them, which means roughly 1 in 20 perfectly normal results sits outside by definition. A single flagged value in an otherwise normal panel is common, and the pattern across related markers matters far more than any one number.

These are the things that move the number without anything being wrong, and they're worth knowing before you worry about a result.
- Alcohol
- Gamma GT is more sensitive to alcohol than most liver markers, which is exactly why it's used. It can stay raised for weeks after stopping.
- Prescribed medication
- A number of common medicines raise it without causing any harm. Tell your doctor what you take.
- Fatty liver
- Increasingly the most common reason for a mildly raised gamma GT in people who drink little or nothing.
- Reading it alone
- The single biggest mistake. Raised gamma GT with a normal alkaline phosphatase means something different from both being raised.

Gamma GT is sensitive enough to move early, which makes it useful for exactly the thing a standard range ignores: a value that has climbed from the bottom of the range to the top is still called normal, and is still a change worth understanding.
The functional question is not whether it is out of range. It is whether it has moved, in which direction, and alongside what else.
